I did a quick look through the Norton catalog and it looks like 2485 RPM is the maximum speed for many if not all of the 10" wheels. I could be wrong, but my impression is that this wasn't a shop for students although students often are more attuned to safety than some experienced person who can't be told how to do things. A wheel spinning too fast doesn't need operator input when it decides that it's time to go.

Yea this was an industrial grinder. the wheel was probably 3" wide. It was a massive machine. Don't remember why it happened, it was too long ago. I sure do remember the sound it made. This was in an Industrial Art class.
