Got all the equipment to put one together I think.

I'm going to mount it on the wall on an adjustable TV mount so it will be movable in and out, just wondering if I should position it so its above the headstock to be kind of inline while using the tool through the hole or should I have it so it's beside and parallel with the hollow form.

Above, on or below centerline? The height placement is going to have to be an average of expected use I know.
 
I use a small monitor intended for a backup camera on a vehicle (9" or 10" I think) that's mounted to a short shaft on a magnetic block. It goes on top of the headstock and winds up being about the same distance from my eyes as the piece I'm working on - it's perfect for me. I'll look for a photo...

hollowing setup This is a link to a hollowing setup post from some time back. In the images you can see that the screen is mounted on a tripod. It is easy to position the monitor where it is most convenient based on the piece being hollowed and the type of hollowing equipment used. Also easy to fold up and put away when not needed.
 
Screen position is a personal preference. You have to learn to ignore the spinning blank and just watch the screen, so experiment and see what you like.
 
I use a 20” monitor, it ends up just above and behind the headstock, large enough to see easily. I wanted it inline with the the tool and wood. Tried it off to the side, didn’t like it all.
