I am looking for any type of consent form or disclaimer you have class participants sign, specifically youth ( less than 18 ) who are taking a course. If you have one for adults, I can modify if needed. Thanks in advance for your assistance.

Thanks for reaching out. AAW has a couple of releases we use for youth who participate in the hand-on youth woodturning activities at the in-person symposiums: 1) Release of Liability and 2) Photo Release form. I have attached them to this post. (Disclaimer: Please use these as examples only. AAW cannot guarantee these samples will meet your specific needs or legal requirements.)
If you have any questions or need anything else, please contact AAW at memberservices@woodturner.org. We rely heavily on our volunteer moderators for the Forum and don't monitor it as closely as email. Sending us an email will get you an answer more quickly.
