I am looking for any type of consent form or disclaimer you have class participants sign, specifically youth ( less than 18 ) who are taking a course. If you have one for adults, I can modify if needed. Thanks in advance for your assistance.

Thanks for reaching out. AAW has a couple of releases we use for youth who participate in the hand-on youth woodturning activities at the in-person symposiums: 1) Release of Liability and 2) Photo Release form. I have attached them to this post. (Disclaimer: Please use these as examples only. AAW cannot guarantee these samples will meet your specific needs or legal requirements.)
