Eccentric Face Plate - anyone recognize this?

Does anyone recognize this eccentric FP? I'm not even sure where I got it, but I suspect it was at one of the Ohio Valley Woodturner's Guild symposia. If it came with an instruction sheet, I've lost that. Just looking it over gives me a general idea of how to use it, but there are a couple parts that I'm not sure what their function is. I'd like to see the details and the instructions might be available online - if I just knew where to look! Thanks.

As I understand it, it's a marvelous piece of kit. I'd like one! I've asked Santa for it for a couple years but Santa hasn't delivered thus far.
