I do not have a name or number for the chuck. The key works with a geared action rather than being a hex key.
The chuck is probably an original Super Nova.There was a machinist in the UK that made replacement keys on a limited basis for around $80.You can get a Supernova 2 on sale for around $120.

There was a small company in England selling them pre-covid. Last I heard he was selling to a bigger firm. I will see if I can follow up and find them.
